The initial consultation can take place in person, by phone, or virtually, based on your convenience.

Once we’ve gathered all necessary information, We will create a tailored care plan specifically designed to meet your loved one’s needs. This plan includes detailed recommendations on medical, social, financial, and housing needs, and outlines the services and resources that will best support their well-being. Whether it’s coordinating with medical professionals, arranging for in-home care, or providing financial management services, the care plan will address all aspects of their daily life and ensure they are receiving the highest level of support.

Ongoing Monitoring and Adjustments

Care management is an ongoing process, we will continually monitor in all three areas, including the initial consultation.
