What Is Care Management?

Care management is a professional service that supports aging individuals and their families by coordinating and overseeing personalized care plans. Through assessments, planning, and ongoing management, care managers ensure that all aspects of a person’s health, home, and social needs are met. Care management brings peace of mind by providing expert guidance, connecting families to appropriate resources, and advocating for the individual’s well-being. Whether managing complex medical conditions, arranging in-home care, or helping with daily needs, care management takes a holistic approach to ensure quality of life and dignity.

Who Is Care Management for?

Care management is designed for older adults, adults with disabilities, and their families who may feel overwhelmed by the challenges of aging or managing health needs. It’s ideal for families who live far away from their loved one, have limited time or resources, or simply need guidance navigating the complexities of healthcare, long-term planning, and support services. Care management also serves individuals who want to age in place with dignity and independence, providing them with the tools, care, and oversight they need to feel secure and supported in all aspects of daily life.

What Is the Process Like?

At Palmieri Care Management, our process is designed to make the journey through aging as seamless and supportive as possible. We begin with a comprehensive assessment to understand each client’s unique needs, preferences, and goals. From there, we work together to create a personalized care plan, providing ongoing support and adjustments as situations evolve. This structured approach ensures that every individual receives compassionate and proactive care tailored specifically to them.

Initial Consultation

We start with a one-on-one consultation to discuss your needs, goals, and any immediate concerns. This is a time to ask questions and determine if our services are a good fit for you and your family.

Comprehensive Assessment

During this phase, we conduct a detailed evaluation of physical, emotional, and social needs, as well as any healthcare, legal, or financial concerns. This assessment helps us develop a holistic understanding of the client’s situation.

Personalized Care Plan

Based on the assessment, we create a custom care plan that addresses both current needs and future goals. The plan includes care coordination, recommended services, and steps to promote independence and well-being.

Ongoing Support & Adjustments

As needs change, we provide continuous support, regularly reviewing and updating the care plan to ensure it stays aligned with the client’s health and lifestyle. We’re always available to answer questions, make adjustments, and advocate for the best possible care.

About Me

Cynthia Palmieri, Owner

As a dedicated Aging Life Care Specialist and Geriatric Care Manager with over 45 years of experience, I bring both professional expertise and personal compassion to the field of senior care. Since becoming a Certified Care Manager in 2009, my mission has been to help older adults and their families navigate the complex world of aging with dignity, clarity, and confidence. My approach combines deep knowledge of healthcare, aging resources, and senior living options with a focus on protecting seniors from abuse, neglect, and exploitation, ensuring my clients feel supported, heard, and empowered.

My extensive experience includes 35 years with the State of Florida, where I dedicated 24 years to Adult Protective Services, helping protect vulnerable seniors who had suffered exploitation and abuse. This experience gave me invaluable insight into the challenges faced by seniors and shaped my advocacy-centered approach. I also provide hands-on support with case management, financial oversight, crisis intervention, and family coordination, helping clients age with dignity in the least restrictive setting. Passionate about helping seniors plan for the future, I work tirelessly to protect their well-being, monitor finances, reunite families, and ensure their dignity at every stage of the aging journey.

Testimonials

“Cynthia was my mother’s senior advocate for more than 10 years. Her dedication, caring, empathy, and knowledge are unparalleled, but more importantly, she was my mother’s friend. My brother and I were so fortunate to have Cynthia supervise my mother’s medical and social needs. I have no words to convey how grateful we are for Cynthia.”

“Cynthia, none of us will ever be able to fully express our gratitude to you. How you shepherded all of us through the most difficult time is really quite a remarkable story. You got my mother to trust you and then treated her with remarkable dignity. Thank you for allowing us to sleep at night knowing our loved one was in good hands.”

“Cynthia, you have been a lifesaver to my family and to me. Neither of us could have gotten through the difficulties of the last couple of years without your efforts to make life more fun for us and by giving me the support I need to help my loved one.”
